Описан случай эффективного применения метода клапанной бронхоблокации при остропрогрессирующей форме туберкулеза - казеозной пневмонии, у больного с множественной лекарственной устойчивостью возбудителя туберкулеза.
The article describes a case of effective application of the valvular bronchoblocation method in an acute progressive form of tuberculosis - caseous pneumonia, in a patient with multidrug-resistant tuberculosis pathogen (MDR MBT).

В условиях роста заболеваемости туберкулезом органов дыхания с множественной и широкой лекарственной устойчивостью микобактерий туберкулеза становится более актуальной проблема адекватного лечения впервые выявленных больных. При этом лечение обязательно должно проводиться в специализированном стационаре в строгом соответствии с ҺФедеральными клиническими рекомендациями по диагностике и лечению туберкулеза органов дыхания с множественной и широкой лекарственной устойчивостью возбудителяһ (2014 г.). Одним из эффективных методов в комплексной терапии лекарственно-устойчивого туберкулеза органов дыхания является технология клапанной бронхоблокации (КББ), основанная на создании гиповентиляции в пораженном участке легкого путем введения в просвет бронха эндобронхиального клапана. В статье показан опыт применения технологии в течение 7 лет в НПЦ ҺФтизиатрияһ. Всего проведено 102 клапанные бронхоблокации, из них 29 по квоте на высокотехнологичную медицинскую помощь, отмечается высокая эффективность применения технологии КББ. Приведенные 4клинических примера комплексного лечения демонстрируют высокую клинико-рентгенологическую эффективность применения КББ при инфильтративном, диссеминированном туберкулезе легких и 1 пример с относительно неблагоприятным исходом клапанной бронхоскопии
With increasing incidence of pulmonary tuberculosis with multiple drug resistance of Mycobacterium tuberculosis, the problem of adequate treatment of newly diagnosed patients has become more urgent. In this case, treatment must be carried out in a specialized hospital in strict accordance with the Federal Clinical Recommendations for diagnosis and treatment of pulmonary tuberculosis with multiple drug resistance of the causative agent (2014). One of the effective methods in the complex treatment of drug-resistant pulmonary tuberculosis is the endoscopic bronchial valve placement (EBVP), based on the creation of hypoventilation in the affected area of the lung by inserting an endobronchial valve into the lumen of the bronchus. The article shows the experience of using the method for 7 years in the PhthisiologyResearch Center. A total of 102 endoscopic bronchial valve placementswere performed, with 29 of them on a quota for high-tech medical care; and the EBVPmethodproved to be highly efficient. The given four clinical cases of an integrated treatment demonstrate high clinical and radiological effectiveness of using EBVPfor the infiltrative, disseminated pulmonary tuberculosis; and one casehad a relatively unfavorable outcome of the endoscopic bronchial valve placement
