Over the period from 2006 to 2016, key epidemiological indicators for tuberculosis in the Sakha Republic (Yakutia) have shown trend to decrease. In 2016, incidence and prevalence of tuberculosis in Yakutia were 1.-1.7 times lower than in the Far East Federal District, but above the Russian indicators by 10.1% and 16.6%, respectively. Tuberculosis incidence in Yakutsk has been declining faster compared to the all-republic rate during the last 10 years, but still, in 2016, key epidemiological indicators for tuberculosis in Yakutsk remained higher than over the republic by 28.2-48.3%. Over the study period, rates of multidrug-resistant tuberculosis were observed to grow. In 2016, incidence and prevalence of multidrug-resistant tuberculosis were 1.4 and 1.7 times higher than in Russia, respectively. Although it needs to be acknowledged that for the last 2 years, incidence of multidrugresistant tuberculosis (MDR TB) in Yakutia has been decreasing, and a trend to stabilization of MDR TB prevalence has been observed, since 2013 and on. Increase rates of incidence and prevalence of MDR TB in Yakutsk have been ahead of all-republic rates, and in 2016, were 2 and 1.6 times higher, respectively, without any trend to decline. Based on comprehensive assessment of the epidemiological situation in Sakha Republic (Yakutia) over the period from 2006 to 2016 with the use of aggregate measurements, epidemiological situation was at worst in 2011, and 2016 was the year with the most favorable TB situation. Churapchinsky district showed optimal epidemiological situation for TB in the years 2006 and 2016, while Yakutsk continues to have the most unfavorable situation with TB

Rates of multidrug-resistance (MDR), including extensive drug-resistance (XDR), were analyzed in new cases of pulmonary tuberculosis (TB) with positive microscopy and/or culture tests, for the Sakha Republic (Yakutia), Yakutsk, rural districts, and for 5 geographical zones composing the territory of Yakutia, over the period from 2012 to 2017. During the study period, we observed unfavorable trend to increasing proportion of pulmonary tuberculosis patients with primary MDR in Yakutia (from 23% to 32.8%). The same rates for Yakutsk and rural districts increased by factors of 1.6 (from 22.5 to 36.7%) and 1.2 (from 23.6 to 29.2%), respectively. In the Arctic zone of Yakutia, for the last 5 years, a decrease was observed (by a factor 1.5) in proportion of MDR TB in new patients with pulmonary TB. In the Eastern Yakutia zone, over the period 2016 to 2017, no case with primary MDR was detected. In the rest 3 geographical zones, the named proportion was observed to increase. Over the period from 2011 to 2016, an increase in proportion of multidrug-resistant cases among pulmonary tuberculosis patients with positive microscopy and/or culture (MTB+) has been observed in Ust-Aldansky district of the Sakha Republic (Yakutia). Proportion of multidrug-resistant (MDR) cases among newly identified patients increased by a factor of 1.2 (from 27.3% to 33.3%), but remained below the all-republic level (34.6%). It should be bared in mind, that in most (68.4%) patients with MDR, resistance is, as a rule, to 3-5 or more antitubercular agents. Effective outcomes in patients, who were started on chemotherapy regimen IV during the period from 2011 to 2014, were observed in 64%, while 4% had treatment failure. We observed high rates of treatment interruptions (12%), mortality from tuberculosis (12%), and proportions of cases transferred out to Directorate of Penitentiary Service (8%), which was attributed to the majority of patients with MDR belonging to socially marginalized population groups (65.8%) often associated with treatment non-compliance and defaulting from regular medical check-ups. Successful treatment outcome in patients with MDR largely depends on well-orchestrated work of all relevant agencies, and primarily, must be supported by heads of local administrations and active members of communities in each of the districts, ‘naslegs', or settlements. All these efforts should be aimed at making sure to hospitalize these categories of patients for motivated long-term treatment

During the COVID-19 pandemic in the Russian Federation associated with restrictions and quarantine, in order to ensure the quality and availability of tuberculosis medical care to the population of the Republic of Sakha, it became necessary to improve and search for new organizational forms of out-patient care. The objective of the study: to analyze the use of new technologies in out-patient care for tuberculosis patients in the Republic of Sakha (Yakutia) during the COVID-19 pandemic. Result: Scientific Practical Phthisiology Center introduced the following information and digital technologies to ensure the availability and quality of medical services to the population of the republic during the COVID-19 pandemic: Cloud Polyclinic, Telemedical Consultations, Radiological Information System of APK ArchiMed , Video-Controlled Treatment at Home, and Hospital at Home. All technologies have demonstrated their efficacy for remote medical consultations and contacting patients online. Based on practical experience, these technologies provided the following: improvement of medical care provided to the population through consultations, maintaining good contact with the patient, monitoring patients' well-being, and drug in-take. They assured compliance with general federal and regional anti-epidemic recommendations to prevent the spread of the new coronavirus infection

The fact that the International Bank for Reconstruction and Development project "Prevention, Diagnosis, and Treatment of Tuberculosis and AIDS" and the Global Fund program "Development of treatment strategy for the Russian Federation’s population susceptible to tuberculosis" were implemented in 2006-2008 has had a positive impact on the efficiency oftuberculosis control in the Republic of Sakha (Yakutia). Thus, there have been increases in the number of first detected with a positive result of sputum microscopy for Mycobacterium tuberculosis detected in primary health care facilities from 25.2 to 30.0%, in that of an effective course of chemotherapy in patients with pulmonary tuberculosis verified by sputum microscopy and clinical and X-ray studiesfrom 76.9 to 81.2%, in the rates of bacterial excretion cessation and decay cavity closure in the first detected tuberculosis patients and the patients registered last year from 80.8 and 76.0% to 84.4 and 82.5%, respectively; the rate of chemotherapy courses being discontinued by patients have decreased from 6.2 to 3.7%; HIV-infected patients have shown higher tuberculosis detection rates for tuberculosis

The objective of the study: to determine the role of polymorphic variants T6000A (rs2430561), T8360C (rs2069718) of the IFNG gene in the development of tuberculosis in the population of Yakut nationality. Subjects and methods. The case-control study was conducted. The subject for the study was DNA samples from 172 tuberculosis patients (the case group) and 367 people with an excluded diagnosis of tuberculosis (the control group). All participants of the study belonged to Yakut nationality. For genotyping, DNA was isolated from venous blood. The genotyping was performed by real-time polymerase chain reaction. For statistical processing, Pearson's χ2 test and odds ratio with 95% confidence intervals were used. The results of the study showed that the distribution of genotypes of polymorphisms T6000A (rs2430561), T8360C (rs2069718) of the IFNG gene in the case and control groups corresponded to the Hardy - Weinberg equilibrium. There was no statistically significant difference in frequencies of alleles and genotypes of polymorphisms T6000A (rs2430561), T8360C (rs2069718) of the IFNG gene in the patients from the case and control groups. Conclusions. The results of the study did not reveal the association of alleles and genotypes of the rs2430561 and rs2069718 polymorphic variants of the IFNG gene with the risk of developing tuberculosis for people of Yakut nationality

Drug-resistant M.tuberculosis (MTB) and drug-resistant nonspecifi c fl ora are highly frequent in destructive pulmonary tuberculosis complicated with purulent endobronchitis. Inhalation therapy with kanamycin dissolved in ammargen supported by chemotherapy allows to achieve the 50,0% isolation cease rate of polyresistant populations of MTB and nonspecifi c fl ora, and to achieve the 75,3% cure rate of draining purulent endobronchitis. Effi cacy data are confi rmed by the results of bacteriologic and biochemical studies, as their results suggest the existence of mutually potentiating bactericidal effect produced by combination of kanamycin with silver ions, and have potential for further development of the novel conception of inhalation therapy
