Environmental pollution first of all affects on health of the children’s population and delays of intellectual and physical development and congenital uglinesses can become the reason of decrease in immunity, synchronization of various diseases. Among various factors of external environment influencing health of the population, a special role is played by pollution of atmospheric air and water sources of drinking water. The city of Yakutsk is the unsuccessful territory, first of all, on sanitary chemically indicator of quality drinking water from a distributive network. In the conditions of lack of water treatment constructions only water disinfecting is carried out by liquid chlorine that conducts to excess education in a chloroform water supply network as product of compound of residual chlorine with organic compounds. Analyzing structure of incidence of children of the city of Yakutsk it should be noted that diseases of a gastrointestinal tract take the 2nd place after diseases of respiratory organs, there is growth of diseases of bone and muscular and urinogenital system

Evaluation of some risk factors for chronic noncommunicable diseases in patients with gastroesophageal reflux disease and metabolic syndrome. Examined 140 patients with gastroesophageal reflux disease (GERD) were hospitalized in the Republican hospital ө 2 and the Yakutsk City Hospital. Depending on the presence of metabolic syndrome (MS) and ethnicity patients were divided into 3 groups: the main group - Yakuts with GERD and MS, comparing the group I - with Yakuts GERD, comparison group II - Russians with GERD and MS. All patients underwent a complete clinical examination with questionnaires developed by the map of the subject approved by the local ethics committee. Statistical processing and analysis of data used IBM SPSS Statistics 19. The majority of patients with GERD and MS (Yakut and Russian) had higher education and were engaged in intellectual work. Yakuts with GERD without MS were more likely to mean less and higher education, as compared with the main group. Assessment of tobacco use in the treatment groups showed no difference. However, the index of smoking in patients with GERD and MS Russian nationality was higher. Regular alcohol consumption is observed in the group of Yakut with GERD without MS. Patients with GERD and MS, regardless of ethnicity, lead a sedentary lifestyle

The article is devoted to actual problems of Pediatrics and Infectology. Despite significant progress in the treatment of hepatotropic viral infections has not yet developed reliable methods of treatment of this pathology. Peg-interferons are the basis etiological treatment of chronic hepatitis. In children, the use of these drugs was permitted, only in 2011. The study group included 50 children and adolescents with chronic hepatitis treated with antiviral therapy. The first group included 24 patients with chronic hepatitis B. The second group included 26 patients with chronic hepatitis C. The study used clinical, immunological methods, technology in molecular biology and instrumental methods of research. The results obtained were processed using statistical methods binary regression and survival analysis. Our experience in the treatment of chronic hepatitis B in children, allows you to identify the most effective treatment regimen. As a result, long-term follow-up after treatment identified factors that influence the effectiveness of treatment and the preservation of the results. Positive predictors of effectiveness causal treatment of viral hepatitis are immunological parameters, and in chronic hepatitis C - the ethnic factor. According to our data patients effectiveness of treatment of chronic hepatitis B does not exceed 30 %, whereas in chronic hepatitis C was 80 %. Duration of preservation of stable remission after closure of antiviral therapy, in patients with chronic hepatitis dependent on sex of the patient and the viral load prior to treatment. The implementation in practice of the standard treatment of chronic hepatitis C, comprising a combination of peg-INFα-2b and Ribavirin, allowed to achieve maximum effectiveness of antiviral therapy. Accounting predictors of treatment effectiveness and the preservation of the results will more accurately predict the long-term results of antiviral therapy in children and adolescents with chronic hepatitis

Venous stroke is one of the rare forms of acute cerebral circulation disorders. The disease develops due to the thrombosis of the brain veins or sinuses. The venous stroke causes are conditions associated with thrombophilia including the antiphospholipid syndrome, pregnancy, sepsis, oral contraceptive use, malignant neoplasms and other diseases. In some cases, the venous stroke cause remains unclarified. The venous stroke etiological factor establishing requires a multifaceted examination, including genetic screening for congenital thrombophilia. This article presents the clinical case of the venous stroke in a young patient. The disease developed in the postpartum period. The clinical manifestations were characterized by the presence of cerebral, meningeal symptoms and convulsive syndrome. The laboratory examination revealed thrombocytosis, hypochromic anemia. The brain magnetic resonance imaging with venography diagnosed the sagittal sinus thrombosis and small area of cytotoxic edema. Examination of somatic status did not reveal the signs of systemic, infectious, oncological diseases. The genetic study was carried out to identify the polymorphism variants, including 677 C / T of MTHFR gene, 1298AC of MTHFR gene, 66 AG of MTRR gene, 2756 AG of MTR gene, 20210 GA of F2 gene, 1691 GA of F5 gene. This genetic screening did not reveal the risk factors for congenital thrombophilia. We carried out the anticoagulant, antiplatelet, anti-edematous, neuroprotective therapy, and the neurological symptoms were regressed, thrombocytosis index decreased, the diameter of the sagittal sinus thrombus decreased. In this clinical case, the cerebral sinus thrombosis developed due to the changes in the hormonal status and blood coagulation properties on the background of pregnancy and childbirth. In clinical practice, it is necessary to pay special attention to the possible risks of cerebral sinuses and veins thrombosis during pregnancy and postpartum period. In addition, it is necessary to examine the carriage of genetic congenital thrombophilia risk factors in young venous stroke patients

The Sakha Republic (Yakutia) is one of high-risk regions in morbidity of B, C, and D viral hepatitis. The assessment of cirrhosis and hepatocellular carcinoma risk factors and ranking of them in Yakutia is a serious medical problem. Based on studying of long-term morbidity of chronic B, C, and D viral hepatitis and their outcomes, the role of HBV and HDV genotypes and the ethnic group had been determined. We destablished the fact of viruses replication in cirrhosis and hepatocellular carcinoma in conditions of unfavorable social and climate factors, genetically determined acetaldehyde increased level due to alcoholdehydrogenase (ADH) and aldehyddehydrogenase (AlDH) among indigenous people of Yakutia, which requires targeted therapeutical measures. We identified the districts of Yakutia, most affected by hepatitis B, C and D viruses with progressive course of the disease to cirrhosis and cirrhosis of the liver, as well as the genotypes of hepatitis B and D viruses most likely leading to primary cancer of the liver. Another result is that combined mixt-replication of hepatitis viruses is a risk factor for the development of primary liver cancer

Obesity in children and adolescents is one of the urgent problems of modern health. The urgency of the problem is due to a steady increase in the number of patients with obesity, involvement in the pathological process of vital organs. Clinical changes on the part of the digestive organs in patients with obesity can be very diverse, characteristic is the presence of comorbidity, which further exacerbates the metabolic syndrome. The article shows a close relationship between obesity and the defeat of the digestive system in children and adolescents of the Sakha Republic of Sakha (Yakutia). In the present work, the nature of lesions of the digestive system was analyzed in 250 children and adolescents with overweight and obesity aged 3 to 18 years, according to the materials of the Department of Pediatric Endocrinology and Gastroenterology and the Consultative Polyclinic of the State Agency of the Sakha Republic of Sakha (Yakutia) RHB No. ө 1- NCM. All children underwent a standard examination, including a history, clinical examination of organs and systems, laboratory and instrumental diagnostics (general and biochemical blood tests, urinalysis, hormone analysis, ultrasound of the abdominal cavity, fibroesophagogastroduodenoscopy (FEGDS), examinations of specialists according to indications). The prevalence of overweight and obesity in boys was established, the predominance of the adolescent age in the group of children surveyed. The connection of morbid obesity in children of the indigenous population of the SRRS (Y) is noted. It was established that obesity in children in the structure of diseases of the digestive system is dominated by liver and bile duct disease. With the increase in the degree of obesity, the frequency of non-alcoholic fatty liver disease increases. Steatosis of the pancreas was found with the same frequency in all groups in children, both overweight and obese. The diagnostics of chronic inflammatory demyelinating polyneuropathy (CIDP) is a difficult task, as the classical diagnostic criteria are not specific enough, the clinical picture of the disease is polymorphic, and there are no reliable biomarkers of this disease. The diagnosis is based on a characteristic clinical picture and is confirmed by the detection of the demyelinating type of peripheral nerve damage in electroneuromyography or nerve biopsy, as well as excluding other diseases that may be accompanied by a similar clinical picture. Currently, the importance of lumbar puncture in the diagnostics of CIDP is being questioned, since not all patients, especially at the outpatient level, are undergoing this procedure, and typical protein-cell dissociation in approximately 40 % of patients may not be detected, especially in the first 2 weeks from the moment of the disease. Therefore, in the diagnostics of CIDP, especially its atypical forms, they introduce such methods as computerized pallestesiometry, ultrasound examination of peripheral vessels, magnetic resonance imaging of the nerve roots, and blood testing for antibodies to viruses tropic to nervous tissue. The article deals with the clinical case of a CIDP patient, who does not have a typical protein-cell dissociation in CSF, however, other paraclinic data, characteristic of inflammatory polyneuropathies, are manifested

To this day, the treatment of patients with the end stage of chronic renal failure (ESRF) remains to be one of the most serious problems of clinical medicine. Despite certain difficulties, the cost of kidney transplantation is much lower than the cost of patient’s treatment with dialysis. Transplantation of the organ becomes the only chance for patients to regain their normal lives; however, providing patients with donor organs is particularly difficult. Although some moral and ethical aspects of transplanting human organs raise a number of questions in our country, it is necessary to choose the cadaveric kidney transplantation as the most preferred method for treating ESRF patients. As of today, there are 400 patients in the Sakha Republic (Yakutia) on hemodialysis, and the need for replacement therapy grows annually. Since 2001, a certain experience of kidney transplantation has been already accumulated in Yakutsk, with 85 living related donor transplantation having been performed. A new stage of transplantology in Yakutia - cadaveric transplantation - began in 2016. This was preceded by a number of preparatory stages: significant work was done by the Ministry of Health of the Sakha Republic (Yakutia); an appropriate legislative framework was prepared at the Federal level; interaction between high-tech giants of the republic’s public health was synchronized; new equipment was purchased; specialists in detecting brain death with the use of modern equipment and international protocols were trained. Certain difficulties with the related donor transplantation open up prospects for cadaveric transplantation. However, is the society ready for this? The article describes the chronology of unique transplantations of kidneys, liver, cornea in Yakutsk and the attitude of the society towards such surgeries. The survey showed the need to continue educational work in the most important issues of modern medicine and, in particular, transplantology. This problem is becoming relevant in the professional training of future doctors in this field, and the role of student scientific societies and subject Olympiads cannot be overestimated
